On June 11, 2026 at approximately 8:50am, Licensing Program Analysts (LPAs) Quimbo and Tse, conducted an unannounced, pre-licensing visit. LPAs met with licensee/director, Sharon Wong, and explained the purpose of the visit.

Program is an already licensed preschool program under license number 414004105. Program requested to add a toddler component. Due to requesting an additional age component, a single license has been generated and the existing license number 414004105 will be consolidated to the single license number 414005343.

Program is requesting to be licensed for a total capacity of 90 children: 12 toddlers (18 months to 36 months) and 78 preschoolers (2 years to entry into first grade). The fire clearance was approved on February 17, 2026 for 90 children.

The center will operate Monday through Friday 7:30am to 6:00pm. LPA toured and measured the on-limits areas and classrooms located at the facility. The facility is a multi-level facility that includes two levels. Day care use for children will be located on the ground level of the facility only.

The toddler program will operate in classroom #1 only. The preschool program will operate in classrooms #2, 3, and 4. Present during today's visit included 52 preschool children and 9 staff.

The indoor toddler measurements contain a total of 634.35 square feet which will accommodate the request for 12 toddlers.

Toddler Outdoor measurements are:
The toddler outdoor measurements contain a total of 951.84 square feet, which will accommodate the request for 12 toddlers.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

Preschool Classroom measurements are:
The indoor preschool measurements contain a total of 2,425.02 square feet which will not accommodate the request for 78 preschoolers.

Preschool Outdoor measurements are:
The preschool outdoor measurements contain a total of 5,856.68 square feet which will accommodate the request for 78 preschoolers.

There are a total of 6 sinks, 6 toilets, and 1 urinal for children's use. There is a shared restroom between the toddler and preschool children next to the facility's lobby. There is an additional preschool restroom in preschool classroom #2 that is exclusively for preschool use. LPAs observed toilets and sinks to be in working condition.

These are sufficient to accommodate the requested total capacity of 90 children. There is a diaper changing table in the shared restroom for diapering children that is within arm's reach of a sink. Diaper changing table is equipped with moisture resistant material with sides of the changing table at least 3" high. Diapering sink is not used for meal preparation or dishwashing.

The child care center was observed to be clean, sanitary and in good repair. Floors of rooms have a surface that is safe and clean. A comfortable temperature for children is being maintained. Furniture and equipment were observed to be maintained in good condition and free of sharp, loose or pointed parts. Tables and chairs were observed to be age appropriate. Classrooms include areas for dramatic play, art, puzzles, and circle time.
Janitorial services, including vacuuming of rugs in classrooms and mopping floors, occur nightly. Napping cots will be utilized for preschool and toddler children. LPAs reminded licensee that children's sheets and blankets are to be stored so that they are not touching each other. Per licensee, sheets are sent home weekly for laundering.

LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at:https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep, as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/, and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Facility is equipped with multiple first aid kits, fire extinguishers, fire sprinklers and smoke and carbon monoxide detectors. Per licensee, detectors are combination smoke and carbon monoxide detectors. LPAs were unable to test detectors during visit due to placement in high ceilings. LPAs advised licensee to install a separate carbon monoxide detector in facility to be tested for a future visit.

When medications are on site, they will be stored in the classroom children will be enrolled in. There are refrigerators in each classroom, if needed. Licensing documents are posted in the main entrance of facility. Children are signed in and out of program with written signatures.

Toddler and preschool outdoor areas are physically separate from one another. LPAs reminded licensee that toddler outdoor area is exclusively for toddler component's use. LPAs reminded licensee that preschool outdoor area is exclusively for preschool component's use.

LPAs observed outdoor areas to be fully fenced and enclosed. There are sufficient outdoor toys and materials in both outdoor areas. Per licensee, toys and materials are sanitized quarterly. LPAs reminded licensee to sanitize materials as needed. There are sandboxes that are covered when not in use. There is sufficient shade for both the toddler and preschool yards. There are stairs in the preschool yard that lead to the off-limits second level, which were observed to be barricaded. LPAs did not observe any pools, spas or bodies of water on site.

Children bring water bottles from home. Water bottles are labeled with each child's name. Drinking water is provided from water fountains.

For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP). Lead testing for facility was completed January 2024 and is within the 5.5ppb threshold.

LPAs reminded licensee that all materials and surfaces accessible to children, including toys, shall be free of toxic substances. Isolation area will be in the lobby, separate from other children in care. Program does not provide a food service at this time. Food will be provided from home. LPAs observed children's personal items to be individually labeled with their individual names.

Licensee was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

Licensee was reminded toddler component cannot exceed groups of 12 children. Licensee was also reminded written authorization from child's authorized representative is required prior to enrollment in toddler component. Licensee is aware there shall be no co-mingling between multiple components.

Based on square footage, maximum capacity facility can be licensed for is 81 children total: 12 toddlers (18 months to 36 months) and 69 preschoolers (2 years to entry into first grade).

Prior to licensure, following is to be completed:
-Updated Application for a Child Care Center License (LIC200A) with updated capacity.
-Updated fire clearance with updated capacity.
-Set up of cleaning supplies for diaper changing station in shared restroom.
-Repair of fence under stairs in outdoor play area.
-Repair of fence behind tree in outdoor play area.
-Disposing of broken items in storage area in preschool outdoor yard.
-Removing toys and materials around electrical utility box.
-Replace fence surrounding electrical utility box.
-Maintaining compliance with warning label on electrical utility box (8 feet clearance in front of box and 3 feet clearance on either side of box, clearing of shrubs around electrical utility box).
-Cleaning of sandbox and deck in toddler yard.
-Repair of holes on deck in toddler yard.
-Submission of shared restroom waiver request.
-Management approval (waiver).
-Centralized Application Bureau (CAB) final approval of application.

Upon license approval of facility number 414005343, facility license number 414004105 will permanently close.
