25 | On December 23, 2021, Licensing Program Analyst (LPA) Jaime Vado conducted an unannounced annual infection inspection. Upon arrival, LPA observed signs on the front door. LPA were greeted by a caregiver and was told that the administrator was on her way to the facility. LPA explained the purpose of the visit. LPA was not screened at the entry point but was offered to sign the visitor log. LPA observed that the log had temperatures for visitors but LPA temperature was not taken.
LPA toured facility and grounds. Two staff are on site wearing masks. Administrator arrived and was wearing a mask. No accessible bodies of water or fire safety hazards observed. Infection control practices are not present: entry procedures, daily monitoring for residents and staff, and 30-day PPE supply. Bathrooms are equipped liquid soap. There was COVID19 signs throughout the facility but LPA advised on adding more face covering and social distancing signs. Majority of the signs or on the front door. Hand washing signs are placed through out the facility but not social distancing and face covering signs. The facility was not able to provide any documentation for the daily COVID-19 screening for the residents and staff members as they discontinued at some point in maintaining this practice and records. According to administrator, all staff and residents are fully vaccinated. LPA observed vaccination card of staff and residents. Medications, toxins and sharps are stored appropriately and inaccessible to residents, and a comfortable temperature is maintained, lighting is sufficient for comfort. First aid kit is present. Fire extinguisher are in good repair last inspected in July 2021.
LPA is advising the following: trash cans with lids, continuation of daily resident and staff checks regarding COVID symptoms and temperature log, adding COVID signs through out, and obtain additional PPE.
This report is reviewed and discussed with the Administrator. |
