On June 20, 2023, Licensing Program Analyst (LPA) Komal Charitra conducted an unannounced case management visit to follow up on the visit conducted on 6/16/2023. LPA met with Caregiver, M.Villones and explained the purpose of the visit.

On 6/13/2023, Code Enforcement Officer notified CCL and Licensee in relation to the violations that were observed during the inspection made on 6/1/2023. Code Enforcement Officer inspected the altered area and found three violations present during inspection. According to code enforcement officer, the office area is a non-habitable space and should not be used for sleeping purposes.

During the visit, LPA observed staff members utilizing the bedroom on the second floor to accommodate 4 live-in staff. In addition, LPA observed the second floor office area that had constructed makeshift walls to be partly removed, however two beds were observed in the second floor hallway and one staff was observed sleeping on the bed located in the hallway.

According to the caregiver, the facility is still working on removing the walls and the beds and will be done by today.

Deficiency of the Residential Care Elderly California Code of Regulations, Title 22, Division 6 is observed and cited on a LIC809D. Failure to correct the deficiencies may result in civil penalties.

Report is reviewed with Caregiver and a copy is provided with appeal rights. Caregiver refused to sign.

Based on observations conducted, facility has installed makeshift walls in the open office area on the second floor without a proper building permit. In addition, on 6/1/23 San Mateo Code Enforcement Officer inspected the facility and indicated that there were violations observed and the facility office area on the second floor is non-habitable and should not be used for sleeping purposes.
