On this day, Licensing Program Analyst (LPA) conducted an unannounced complaint investigation visit in order to deliver the findings regarding the allegations. LPA met with administrator Monica and explained purpose of today's visit.

During the course of the investigation interviews were conducted and review of pertninent documents were made. Due to the COVID pandemic at the time of the allegations several staffing impacts took place in regards to staffing changes, temporary agency use utilization, and multiple staff leaving, the stafffing levels fluctuated within the facility this was something the facility was not able to control due to the pandemic. Staff tended to residents the best they could to meet residents needs. At no point was it found that there were no staff present. These allegations are unsubstantiated.These allegations are unsubstantiated.

Based on these observations, the above allegations are UNSUBSTANTIATED.
Although the allegations may have happened or is valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the above allegations are unsubstantiated at this time.

On this day, Licensing Program Analyst (LPA) conducted an unannounced complaint investigation visit in order to deliver the findings regarding the allegations. LPA met with administrator Monica and explained purpose of today's visit.

During the course of the investigation interviews were conducted and documentation was reviewed. It was discovered that the increase in rae was due to annual increases that are agreed upon signing the admission agreement. This increase is outlined in the agreement at time of admission and such notification was sent to the responsible party prior to the increase. Additionally care needs changes with the resident which warranted the price adjustments. Notification is provided to responsible party. This allegation is unfounded.

This agency has investigated the complaint alleging, facility did not notify resident of rate increase. We have found that the complaint was unfounded, meaning that the allegation was false, could not have happened and/or is without a reasonable basis. We have therefore dismissed the complaint.
