25 | On December 21, 2020, Licensing Program Analyst (LPA) Ashley Boothe conducted an announced case management visit via Microsoft Teams at 1pm with Executive Director Martha Estalano and Office Manager Susan Ladeza. Line list was reviewed. Resident one will not be released on Wednesday as previously noted. No changes noted for other residents. Meeting time changed to 2pm tomorrow and LPA emailed updated invitation.
Residents observed on a door visit with family members in the lounge. Kitchen observed to be clean and stocked with food. Stickers were observed to include preparation and expiration dates. Residents observed in dining area and activity room wearing masks and maintaining social distancing. Screening logs for two staff and phone system technician visitors were observed to be completed by designated staff on the LIC500 assignments. LPA observed two locked exit doors on floor one and floor two without a working alarm. Martha stated Travis contacted Bay Alarm, no work order for the exit door alarms has been completed. Signs posted were inaccurate instructions to staff lock the door after 7pm and an alarm will sound after midnight. Martha removed signs, LPA recommended checking all exit doors for updated signs to be posted. LPA observed consolidated MAR's for two residents started on 12/18/2020. Separate binder for Centrally Stored Medications log. Staff restroom locked and stocked with soap paper towels and hand washing sign and touchless trash can. LPA observed no soiled linens on the floor in the laundry and locked chemical storage. LPA interviewed two staff and they were able to provide information on disinfection practices, one Zep bottle observed without a sit time on it. Dry food storage observed organized and no improperly stores foods or chemicals in food storage areas. LPA observed break area chairs were less than 6 feet with three chairs at a table, Susan moved them. LPA observed stand up meeting communication log for 12/18/2020 to included information on Zep, hourly log screening assignments for 4pm, 7am, and 11pm, supplies, and resident updates. LPA observed stand up meeting communication log for 12/20/2020 where Travis stated notes on donning and doffing procedures but did not provide follow up recommendations. LPA recommended to include what the communication is to staff questions to ensure consistency in communication to all staff. LPA observed two residents’ rooms and laundry baskets have been lined with mesh bags. Hour logs for the two residents were filled out for 12/21/2020.
No deficiencies were observed during today’s visit. An exit interview was conducted with Martha. A copy of this report was provided to Martha via email, due to COVID-19 precautionary measures, with a "read receipt" to verify the LIC 809 was received. Martha is print out the report and fax a signed copy to LPA at 916-263-4744 or email to LPA at ashley.boothe@dss.ca.gov. |
