On December 22, 2020, Licensing Program Analyst (LPA) Ashley Boothe conducted an announced case management visit via Microsoft Teams at 2pm with Executive Director Martha Estalano.and Office Manager Susan Ladeza.

The team discussed ED was reported to be on vacation outside of the Bay area region. LPM provided recommended to follow up with LHD for guidance on his return to the facility and visiting a community with widespread COVID transmission. Line list was reviewed, no changes noted. Requested document for resident nurse visit on 12/21/2020. Requested update Facility Sketch with resident rooms updated by end of day 12/30/2020. Requested Hourly logs for from 12/19 to 12/21 by end of day 12/22/2020. Requested documents of fire drill practices sent to RO by close of business 12/23/2020. Requested documents stand up meeting notes from 12/22/20202 sent to RO by end of day 12/22/2020. LPA provided PIN via email 20-43-ASC-CCR: Coronavirus disease 2019 (COVID-19) infection control guidance for celebrations and outings to review practices for group activities by end of day 12/23/2020.

Staff discussed topics of stand up meeting today including: donning and doffing of PPE and Exit doors now have an alarm so that staff cannot enter though those doors anymore. NOC is conducting hourly checks and residents are sleeping well. Reminded staff to redirect residents to comply with universal COVID precautions of social distance and wearing masks. LPM recommended training staff on exit procedures and to not turn off alarms. LPM recommended mock training and provide specific training to get comfortable with donning and doffing in isolation area. Observed Fire Exit door on floor two alarmed and signs posted to keep the door locked, exit only. Martha stated Travis fixed it yesterday. Martha stated the activities calendar posted in the resident rooms and in the hallway. Not observed and Martha stated there is no activities for residents since COIVD time but they go group activities with social distancing in the morning including exercise, stretches, singing, current events and afternoon including vibrator machine, cycling, and movies. Observed hourly log for resident one following repositioning every two hours and a sign posted on door to follow line list instructions residents care on outside of resident’s room. Isolation area was observed, room partitions have arrived and Martha will request Travis to get them put up asap. Due to poor wifi connectivity today no other areas were able to be observed. Martha stated the technicians were working on the facility’s upgrade today.

An exit interview was conducted with Susan. A copy of this report was provided to Susan via email, due to COVID-19 precautionary measures, with a "read receipt" to verify the LIC 809 was received. Susan is print out the report and fax a signed copy to LPA at 916-263-4744 or email to LPA at ashley.boothe@dss.ca.gov.
