On January 11, 2021, Licensing Program Analyst (LPA) Ashley Boothe conducted an announced case management visit via Facetime 2pm with Infection Preventionist Ricardo Aban and executive Director Martha Estalano. Census 24. Resident in hospital still not ready for discharge from hospital. Martha stated no other incidents to report. One perspective resident not ready to move in yet. COVID testing on 1/11/2021 5 staff, 3 residents and perspective resident and his responsible party

Ricardo stated COVID vaccine for 20 staff out of 25 is scheduled to go to SMCPH clinic. LPA provided technical assistance to continue to follow universal precautions until there is updated CDC guidance.

LPA reviewed PIN 20-48-ASC, Mitigation Plan LIC808, and Dementia addendum to be submitted to LPA by 1/24/2021. Stand up meeting notes on 1/11/2021. Stand up meeting log was reviewed, reminders, vaccinations, residents needs discussed. Documents included staff initials after they have read it and the communication is working so all staff are receiving linear communication. Activity calendar stated 2pm celebrating resident’s birthday. LPA observed 6 residents in lounge and staff was reading life story and cupcakes were available. Staff and residents observed to maintain social distance during birthday celebration. Kitchen observed staff preparing fried chicken for dinner. LPA provided technical assistance to secure key for sharps drawer. LPA observed refrigerator temperature to be recorded at 45 F at 12pm but observed at 40 F. LPA provided technical assistance to keep the temperature under 41 F by changing time of the temperature check during a time staff are checking, reducing the amount of food stored, or training staff to kept it closed more often. First floor observed, hourly logs completed for resident room 28 and 27. Common restroom observed stocked with no cloth towels. Exit doors observed on floors 2 and 3 locked and alarmed.

No deficiencies observed on today’s visit. An exit interview was conducted with Ricardo. A copy of this report was provided to Ricardo via email, due to COVID-19 precautionary measures, with a "read receipt" to verify the LIC 809 was received. Ricardo is print out the report and fax a signed copy to LPA at 916-263-4744 or email to LPA at ashley.boothe@dss.ca.gov.
