25 | On January 22, 2021, Licensing Program Analyst (LPA) Ashley Boothe and Licensing Program Manager (LPM) Liza King conducted an announced case management visit via Teams at 1pm with Infection Preventionist Ricardo Aban.
Census 23. 25 with one transferring to a rehabilitation facility and one is hospital. One resident suffered a post injection reaction to COVID-19 vaccination on Friday including symptoms of confusion, elevated temperature, and injury tear on left knee. The facility staffed contacted paramedics, resident refused transport to hospital, paramedics assessed him, and resident was found to be within normal range. LIC 624 will be submitted to LPA. LPA received the Mitigation Plan addendum by the due date 1/24/2021. LPA will review. request changes, and approve once completed.
Ricardo and Martha reported a new admission is coming on 1/25/2021. Resident has been isolated in home, was transferred to hospital for non COVID reasons and will now transfer to the facility. Resident's COVID test resulted negative while at the hospital. Licensee requested Ricardo contact SMCPH to find out if resident will need to go into isolation in the Red Zone with designated care staff or if he can isolate in Room 31. SMCPH’s recommendation was to isolate with designated care staff. One previous instance of resident returning from hospital that did not follow the protocols outlined in Section VI. Returning and Re-admission and New Admissions of Residents in the approved Mitigation Plan. LPM provided technical assistance that by not following the Mitigation Plan the facility is opening itself to liability of exposure if residents are not isolated as the plan outlines. Ricardo and Martha stated they will inform the family and the Licensee to move the new admission to room 17 in the Red Zone and follow the isolation procedures and assignment of care team staff for 14 days as outlined in Section VI. Returning and Re-admission and New Admissions of Residents in the approved Mitigation Plan. Observed Room 47 and restroom with no hand soap in the restroom, facility in the process of moving resident to floor 2 Room 12. Room 48 and restroom, empty, restroom observed no paper towels, Licensee is painting today so the paper towel dispenser was taken off the wall. Room 31 and restroom observed with uncovered trash can. LPA provided technical assistance to replace with touchless covered trash can. Staff restroom observed on floor 2, with hand washing signs, soap, paper towels available. Residents observed maintaining social distancing in the lounge during the window visits. Observed ample and properly stored food supply in kitchen and first floor storage. One item was expired on 1/24/2021 and LPA provided technical assistance and staff disposed of it.
An exit interview was conducted with Ricardo. A copy of this report was provided to Ricardo via email, due to COVID-19 precautionary measures, with a "read receipt" to verify the LIC 809 was received.
