25 | On April 16, 2021, at approximately 1:30pm, Licensing Program Analyst (LPA) Michael Garcia conducted an unannounced case management inspection in response to an unusual incident that the facility had self-reported. Due to COVID-19 pandemic, the inspection was conducted remotely, via video call, with Abbie Apolinario, executive director/administrator.
Today, LPA interviewed the administrator and reviewed some documents.
Administrator shall ensure to email the following documents to LPA within 24 hours of this report:
- Current resident roster.
- Resident 1 (R1)'s Physician's Report.
- Nurses' notes regarding R1 for the past 7 days.
- Staff 1 (S1)'s written statement dated April 15, 2021.
- S1's April work schedule.
- S1's most current training records regarding resident rights.
An electronic copy of the report was emailed to Abbie for signature. |
