25 | On May 17th, 2023, at 1:41PM, Licensing Program Analyst (LPA) Rosie Breault made an unannounced visit for the purpose of conducting a Case Management - Incident inspection. LPA met with facility director Maggie Flores and discussed the purpose of the visit. At the time of the inspection there were thirty-two (32) children and ten (10) staff present.
On 5/01/2023, director contacted Community Care Licensing (CCL) to self-report an incident of a child unattended outside for approximately one (1) to (2) minutes. On 4/28/2023, children in Preschool Classroom No. 4 were playing in large outdoor area. At the time of the incident there were eight (8) staff present for care and supervision in the outdoor area and twenty-nine (29) children. Child was near the far-right corner chain link fence (approximately 4.5 ft in height) playing when child started climbing fence. Teacher (hearin referred as S1) was stationed roughly fifteen (15) feet from child, while another staff member was located closer however observing another unit of children at play. S1 observed child climbing fence and hurriedly rushed over in order to stop child; however, child jumped the fence prior to S1 arrival. Child did not jump off school property but ran over toward rooms 101 and 102. S1 then backtracked around the classroom and intercepted child in front of Room 102. Based on observation, LPA determined S1 was at an appropriate distance, child was in plain view and the area was not obstructed by trees or equipment. LPA observed the area of the incident, interviewed director, and determined child was under supervision by S1 however, the quickness of the child prevented S1 from stopping the incident from occurring. Director advised LPA child’s parent was informed upon pick up and child continues to be enrolled. Director advised a work order was immediately placed for areas of fencing and facility is working to decide if a tarp, plexiglass or other barrier will be most effective. In the interim, the child is being closely monitored during play periods.
No deficiencies were cited during today's visit.
