On December 18, 2023, Licensing Program Analyst (LPA) Giovani Gonzalez conducted an unannounced inspection at the abovementioned Family Child Care Home (FCCH) to close a complaint investigation. LPA met with Licensee Dawn Chapman and informed them of the purpose of the inspection. At the time of the inspection there were 6 children present.

The Department received 2 allegations. The first allegation was that Licensee left day care child unsupervised for an extended period(s) of time. During LPA’s initial visit they observed that there was an infant sleeping in a separate room with the door closed. Parent interviews revealed that this was also the case for other infant children. The second allegation was that the Licensee did not ensure day care child’s hygiene needs were met while in care. Parent interviews revealed that this was the case for other infant children as well. 1 parent provided a video where their infant child’s face was dirty and had various cuts on their forehead.

Based on LPAs observations, interviews which were conducted, documents gathered and/or record review(s), the preponderance of evidence standard has been met, therefore the above allegations are found SUBSTANTIATED. California Code of Regulations, Title 22, Division 12 or Health and Safety Code, are being cited on the attached LIC 9099D.2 Type B deficiencies are being cited.

Exit interview was conducted and report was reviewed with Licensee Dawn Chapman. Notice of site visit and appeal rights were given.
