25 | Licensing Program Analyst (LPA) Rankin arrived at the facility unannounced to conduct a required annual visit at 9:45. LPA was greeted by Administrator Jean Sarmiento and informed them of the reason for the visit. Facility is scheduled to close mid-December, as of the time of visit there is one resident still present.
LPA toured facility with Administrator. The facility is maintained in conformance with state fire marshal regulations. Facility is safe, sanitary, and in good repair. Fire extinguishers were fully charged. Inside and outside passageways are free from obstruction. Residents’ rooms are appropriately furnished with adequate lighting. LPA observed more than two days of perishable and more than seven days of non-perishable food. Food is stored in proper containers in the refrigerator and freezer. Kitchen is clean and sanitary. The washer and dryer are in the garage. The garage is locked. Restroom is clean and sanitary and in operating condition with non-skid shower floor.
Items that could pose a safety risk such as sharps, and cleaners are inaccessible to resident. Common areas are clean and furnished as required. Medications review completed for one resident. The medications are centrally stored and locked in a closet in the kitchen. Medications are labeled and checked for expiration dates.
LPA reviewed resident record for, but not limited to, the following: signed admission agreements, current medical assessments with TB results, and current needs and services plan. File was complete.
If facility remains open past the end of December of 2024 LPA may to a follow-up visit.
