On 4/14/2026 Licensing Program Analyst (LPA) German Negrete made an unannounced inspection for the purpose of conducting/closing three Case Management – incident reports. LPA met with Administrator Maite Estrada and discussed the purpose of the inspection. LPA conducted a walk through inside and out of the CCC. During today’s walk through LPA observed 30 children receiving care and supervision from 7 CCC staff-members .

The following incident is under review and was self reported to the department on 3/2/2026: C1 was at the CCC, outdoor play yard. While walking, C1’s legs gave out, and C1 fell forward, C1s body hit the dirt pit (outdoor play yard). S1 went over to check on C1 and attempted to lift C1 but was unable to do so. S2 came over and observed C1's body shaking. The kitchen cook walked by to check on C1, and C1 began to turn purple. C1 was transported to cottage hospital by the paramedics.

The following second unusual incident is under review and was self reported to the department on 3/26/2026: During a fire drill, C2 was left behind while sleeping in the rainbow classroom. Staff conducted roll call at the "safe area" outside the CCC. S4 noticed that C2 was missing. S3 returned to the classroom , found C2 asleep and brought C2 to the "safe area". C2's absence was not noticed for approximately 5 minutes later.

The following third incident is under review an self reported to the department on 07/17/2025 by Facility Representative contacted Community Care Licensing (CCL) to self-report the following unusual incident : At approximately 9:38AM S5 was in the classroom with 8 children while two staff were outside with 10 children. S5 took six children on a walk and sent two children outside. S6, who was waiting by the ramp outside, was observing the classroom through a window. S6 saw two children in the classroom bathroom, and were unattended. S6 went to bring two children outside where S6 provided care and supervision. Continued on LIC809-C

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

According to Administrator the two children were left unattended for approximately 10 to 15 seconds.

The Unusual Incident Report (LIC624) for the 3/2/2026 incident was received by the department on 3/5/2026. The LIC624 for the 3/26/2026 incident was received by the department on 3/27/2026. Both LIC624s were received in the appropriate time frame as outlined in Title 22 California Code of Regulations. The Unusual Incident Report (LIC624) for 7/17/2025 was received on 7/21/2025 during a Licensing inspection, and all three LIC624s were submitted in the appropriate time frame as outlined in Title 22 California Code of Regulations.

The staff interviews for the 3/2/2026 incident revealed, that once staff observed C1 symptoms, and saw C1's face turn purple, they quickly contacted the paramedics. However there was confusion while communicating the incident to the paramedics over the phone. CCC staff had trouble obtaining the CCC address for the paramedics . Administrator stated there is a new policy in place, where CCC emergency phones(work phones) will have emergency information labeled in back of the phone.

During the departments review of the 3/27/2026, staff interviews were conducted. The staff interviews revealed, C2 was left behind for approximately 5 minutes. The staff interviews also revealed CCC staff were confused during the fire drill and C2 actually was sleeping behind a block shelf(furniture). According to CC staff the aforementioned factors lead to the confusion and to C2 being left unattended without care and supervision.

Today LPA measured the classroom/area where C2 was sleeping and was left behind. Also LPA measured the area/corridor that leads to the front exit, where CCC staff walked and utilized to reach the "safe area" during the fire drill. The distance from the classroom to the corridor/exit measures more than 45 feet(see LIC812).

Today CCC will receive one Type B deficiency Under title 22 California Code of Regulations section 101229(a)(1). Appeal rights were Provided to Administrator. Exit interview conducted and report was read to Administrator Maite Estrada. Notice of site visit was provided.

Notice of site visit was provided as required by Health and Safety Code Section 1596.817 and must remain posted in a prominent publicly accessible area in the center for 30 days, or a civil penalty of $100.00 will apply.

On 4/27/2027 the rainbow fish classroom will no longer be utilized by the CCC. A new blue dolphin classroom and the current sea turtles classroom will be utilized for the preschool children.

Based on observations and self reported incidents, it was discovered on 3/26/26, C2 was left unattended, in rainbow fish classroom, without care and supervision for approximately 5 minutes during a fire drill.
