32 | Through staff interviews, two incidents were discovered which corroborated the allegation of staff mishandling a day care child resulting in a violation of Personal Rights.
Based on LPAs observations, interviews, review of documentation, and evidence gathered, the preponderance of evidence standard has been met, therefore the above allegation is found SUBSTANTIATED.
As a result, one Type B deficiency under California Code of Regulations, Title 22, Division 12 is being cited and can be found on the attached LIC 9099D.
A notice of site visit was given and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days.
