On 05/28/2025, Licensing Program Analysts (LPAs) Bill Billones and Elizabeth Geroge conducted an unannounced inspection to deliver findings related to a complaint received by the department on 02/28/2025. LPAs met with Ariana Melero, Office Coordinator, of the Child Care Center (CCC). At the time of the inspection, LPAs noted 21 children and 5 staff providing care and supervision.

The complaint alleged violations of Personal Rights, specifically a staff member mishandled a day care child, resulting in actions that violated the child’s personal rights.

The investigation included two unannounced inspections. As part of the investigation, LPA reviewed the children’s roster, individual files, unusual incident reports, and the facility’s parent handbook. LPA also conducted interviews with facility staff, the site supervisor, and parents of currently enrolled children. Relevant electronic communications between staff and parents were reviewed.

Through staff interviews, two incidents were discovered which corroborated the allegation of staff mishandling a day care child resulting in a violation of Personal Rights.

Based on LPAs observations, interviews, review of documentation, and evidence gathered, the preponderance of evidence standard has been met, therefore the above allegation is found SUBSTANTIATED.

As a result, one Type B deficiency under California Code of Regulations, Title 22, Division 12 is being cited and can be found on the attached LIC 9099D.

A notice of site visit was given and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days.

Exit interview conducted, appeal rights provided, and report was reviewed with Office Coordinator, Ariana Melero.

Licensee will provide a training on Title 22 Personal Rights to all staff involved in the care of children. Administrator will submit the training curriculum and staff attendance to Licensing Program Analyst Bill Billones via email at bill.billones@dss.ca.gov no later than June 20, 2025.
