On 04/16/2021 at 9:20 AM, Licensing Program Analyst (LPA) Sylvia Mendoza-Ceja conducted an unannounced tele-inspection via Zoom due to COVID-19 State of Emergency in order to conclude the investigation of the above allegation. LPA spoke with Nazish Zaffar, Site Supervisor and explained the purpose of the inspection. Today, there are 18 children in care supervised by the Site Supervisor, two teachers and two aides. The allegation is that an unqualified staff is being counted in the ratio. Investigation included interviewing Executive Director, staff, and some of the parents of children in care; and reviewing staff records. LPA was unable to interview the complainant as no contact information was provided.
-The Site Supervisor disagrees with the allegation. None of the staff interviews corroborated the allegation. None of the parent interviews corroborated the allegation. Records review reveals that all of the teachers are qualified and that a majority of the aides meet the teacher educational requirements. Although these allegations may have occurred, there is not a preponderance of evidence to prove that the alleged violations did or did not occur, therefore, the allegations listed above are deemed UNSUBSTANTIATED.
-Exit interview was conducted the Site Supervisor, via tele-inspection, during which appeal rights were explained. This report along with a copy of the appeal rights and Notice of Site Visit (LIC9213) will be sent to the via email with a read receipt or confirmation of receipt of email, which will act as the Site Supervisor signature. The LIC 9213 Notice of Site Visit shall be posted for 30 consecutive days. Failure to maintain posting as required will result in a $100.00 civil penalty. |
