The purpose of this amendment is to update the administrator's name.

On October 21st, 2022 at 2:28PM Licensing Program Analyst (LPA) Rosie Breault conducted an unannounced Case Management inspection for the purposes of staff file review. LPA met with site supervisor Mayra Lopez and was provided a tour of the facility inside and out. At the time of the inspection there were sixteen (16) children present and (2) teachers.



On October 5th 2022, LPA conducted an Annual Random inspection on this facility and second location (#426212030) located at 2121 De La Vina Street in Santa Barbara, CA. During the inspection of the De La Vina location, director Jacqueline McDonough stated all staff files are house at the State Street location.

Today, LPA arrived at State Street location, this facility, to continue the review of a sampling of staff files. LPA contacted director via phone to inform of inspection. Director stated staff files of teachers working at the De La Vina location, have been moved to that site. LPA spoke with Human Resources (HR) manager, Adriana Servio during today's inspection who confirmed files have been moved to De La Vina; however, the files are locked, director is only staff at this time who has key, and is off for the day. HR manager provided LPA a list of the five (5) staff who work at the De La Vina location. LPA commenced to review one teacher file at State Street location to ensure compliance. Per review, file is in accordance with policy.

There are no deficiencies cited during today's inspection.

Exit interview with site supervisor was conducted.
