13 | The purpose of this amendment is to update the administrator's name.
On August 22nd, 2022 Licensing Program Analyst (LPA) Rosie Breault, made an unannounced inspection to conclude the complaint listed above. Prior to entry, Covid – 19 prescreening questions were asked and no exposure in the home. LPA met with Director Jacqueline McDonough and explained the nature and purpose of the inspection. Director provided LPA a tour of the facility both inside and out. At the time of the inspection there were 18 children present and 5 staff.
The Department (Dept.) obtained allegations that a staff member handled child in a rough manner.
During the investigation, facility provided the Dept. with file records, written statement(s) regarding the allegation, and the Dept. conducted interviews with parents and staff.
On 8/19/2022, Director verbally stated to LPA via telephonic conversation that staff member had been terminated from facility effective 6/25/2022.
Based on LPA’s observation, record / file review, and interviews conducted, the preponderance of evidence standard has been met, therefore the above allegations are found SUBSTANTIATED. LPA observed Director posted the Notice of Site Visit.
This investigation is closed.
Exit interview was conducted with licensee. Notice of Site Visit (LIC 9213) posted during LPA inspection, and must remain posted for 30 consecutive days. Failure to maintain posting as required will result in a civil penalty of $100.00.
