On 03/26/2025, Licensing Program Analysts (LPA) German Negrete and Associate Governmental Program Analyst (AGPA) Francisco Pedroza made an unannounced inspection to deliver findings of the above allegation. LPA met with Program Director Jacqueline Mcdonough, Executive Director Gabriela Garcia, Site Supervisor Richard Gonzales (CCC). LPA explained the purpose of the inspection. During the inspection LPA toured the facility, LPA noted 22 children and 6 staff providing care and supervision.

The Child Care Licensing Regional office received the complaint allegation on 07/23/2024. LPA initiated the investigation 08/01/2024. The investigations included interviews with staff, Family wellness practitioner (FWP), parents of both current and previously enrolled children, as well as LPA observations. LPA also interviewed executive director and prigram Director, former staff and current staff. Additionally, LPA reviewed unusual incident reports from the last 18 months and documents/flyer pertaining to a Latino organization event(mixer). Continued on LIC9099-C

Regarding Allegation Staff left inappropriate items accessible to daycare children. LPA received and reviewed unusual incident reports(UIR) from the previous 18 months. LPA identified a UIR that occurred on 9/26/2023. The following UIR was self reported to CCL on 9/28/2023(See LIC912). Family wellness practitioner and C6 were working together at the center. C6 was not listening to directions by FWP, C6 opened multiple doors at the facility. C6 then ran to the side gate of the center where C6 then entered the back patio of the building. The FWP followed C6 to de-escalate the situation. There was an ice chest in the area(patio) and C6 took a container of alcohol (un-opened) out of the chest and proceeded to show the FWP.

As mentioned LPA interviewed parents of both current and previously enrolled children . From the parent interviews LPA discovered most parents are satisfied with the care and supervision the center staff provides. Additionally LPA interviewed parent(P6) of C6. P6 confirmed the center notified P6 of the aforementioned Unusual Incident that took place on 9/26/2023.

LPA interviewed both Director and Executive Director(E.D). Both the Director and E.D stated the reason there was a Ice chest was due to a social mixer that took place at the center(outside of operation hours). This mixer was hosted by E.D. Also Director stated, during the Unusual incident, staff offered to assist FWP, however FWP refused assistance. LPA interviewed FWP. The interview revealed how FWP did ask for assistance, after C6 opened the first gate/door (See LIC812-C). According to FWP there were a total of 4 gates/doors that C6 opened during the incident.

Furthermore LPA reviewed electronic communications that were exchanged by FWP, and Child Care Director. The electronic communications revealed the exact time line of the UIR(see LIC812). Additional LPA received and reviewed a flyer from E.D. This flyer contained the name of the organization as well as the time and date the event took place. The date listed on the flyer was of

9/25/2023. LPA discovered the date listed on the flyer is false and inaccurate. The 9/25/2023 never existed on the 2023 calendar year. The department discovered a second similar flyer on the Latino Circle organizations face book page. The Facebook flyer had a date of 9/20/2023 (See LIC809- Case Management Report), which suggests the cooler containing alcohol was left on the property for up to 6 days.

Based on LPA interviews, documents collected during investigation, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. California Code of Regulations, (Title 22, Division 12 & Chapter 1), are being cited on the attached LIC 9099D.”



Exit Interview conducted and report was reviewed with Director Jaqueline Mcdonough and E.D Gabriela Garcia.

Notice of site visit was provided.

The CCC implemented a policy for event request and guidelines for clean after events and CCC moved admin staff and supplies to another location to prevent reoccuring issues.
The CCC staff and management will have to watch the two following videos that are listed on the CDSS website:

CCC staff left a ice cooler containing alcohol on CCC property. C6 grabbed a unopened container out of the cooler on 9/26/2023. This posed a potemtial risk to the health and safety of chidlren in care.
