25 | On December 8, 2021 at 3:15 PM, Licensing Program Analysts (LPAs) S. Mendoza-Ceja and D. Thompson conducted an unannounced case management inspection and met with Jordan Brounstein and Anabel Padilla. The purpose of the inspection was to review the Accusation #7921123001, dated May 13, 2021, and the Decision and Order #7921123001 effective June 21, 2021 to ensure Serafin Salmon is not present at the facility. LPAs toured the center inside and outside.
The Department notified the center Serafin Salmon is prohibited from employment in presence in, and contact with clients of, any facility licensed by the Department, certified or approved by a licensed foster family agency, or any resource home. Serafin Salmon has been excluded from all facilities licensed by the Department for the remainder of his life, unless he successfully petition for reinstatement or reduction of penalty pursuant to Government Code Section 11522.
LPA provided a copies of the Accusation #7921123001, dated 05/13/2021, and the Decision and Order #7921123001 effective June 21, 2021.
Anabel Padilla stated Serafin Salmon was hired, but never worked at the center because he failed to obtain the criminal record clearance/exemption with the Department.
LPAs determined Serafin Salmon is not on the premises. |
