On April 9, 2024 Licensing Program Analyst (LPA) Giovani Gonzalez conducted an unannounced Case Management - Incident inspection at the above-mentioned Child Care Center (CCC). LPA me with Director Jacqueline Mcdonough and informed the purpose of the inspection. At the time of the inspection there were 27 children present.

The CCC self reported an incident that occurred on 3/13/2024 where a child (C1) was left unattended briefly by a behavioral support therapist. According to the report the therapist had stepped inside to have their exit paperwork signed without the C1. Staff 1 (S1) then ran out to grab the child.

LPA discussed incident with Director. Director stated that they informed the the therapist that they cannot leave children unattended. Director stated that they also spoke to the therapist's supervisor. Director stated that the therapist's supervisor had already spoken to their staff members of not leaving children unattended. Director stated that the center will no longer allow the therapists to be left alone with children. Director also stated that the C1 was only left unattended for less than a minute. Director also stated that parent of C1 was contacted immediately as well.

LPA is reminding Director that children must have supervision at all times and that when children are present at the center, it is their responsibility to ensure that children always have supervision.
