32 | 10/23/2023, @ approximately 2:30pm licensee noticed C1 was not in the day-care, and approximately 2:40pm, Jose Gomez, licensee's adult son found C1 approximately two (2) house away from the corner of S. Thornburg Street and Ronald Place, Santa Maria, CA 93458. It is approximately 280 feet from the family child care home. Mr. Gomez met with two (2) women, who were with child #1, and had observed C1 alone without supervision.
Mr Gomez stated the women turned over C1 to Mr. Gomez, and advised Mr. Gomez the Santa Maria Police had been contacted of a child wandering in the street unsupervised. The Santa Maria Police arrive to the home at 3:31pm.
The Police Officers went to the licensed day care home to investigate the incident. While at the licensed day care home, the Police Officers observed C1 was had been found and returned to the day-care.
According to CCR, Title 22, Division 12, the following Type A deficiency and Civil Penalty were cited for Care & Supervision. Appeal Rights were reviewed.
Licensee shall post and provide copies of the licensing report to parent/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months. Licensee has been provided a copy of LIC 9058 Applicant/Licensee Appeal Rights and a copy of LIC 9224 Acknowledgement of Receipt of Licensing Reports.
Upon receipt of this report, licensee shall post and provide copies of this licensing report to parents /guardian of children in care at the facility and to parent/guardians of children newly enrolled at the facility during the next 12 months. Licensee to provide LIC 9224 for each child in care and have each parent sign the form that they have received a copy of the report LIC 809 and LIC 809 D.
THIS REPORT MUST BE FILED IN FACILITY FILE AND MADE AVAILABLE FOR PUBLIC REVIEW FOR 3 YEARS. LPA observed the "Notice of Site Visit" posted. FAILURE TO POST THE NOTICE OF SITE VISIT FOR 30 DAYS MAY RESULT IN A $100.00 CIVIL PENALTY.
