25 | On 10/15/25, at 11:45 AM, Licensing Program Analyst (LPA) Matthew Sapien conducted an unannounced annual random inspection of the abovementioned Family Child Care Home (FCCH). LPA met with Tawnie Clark, Licensee of the FCCH, and explained the nature and purpose of the inspection. The LPA, in the company of Licensee, toured the FCCH in its entirety (inside and outside). The home is a 3 bedroom and 2 bathroom single story home. The areas that are accessible for day care children are as follows: living room (main play room), dining room, kitchen, one bathroom, and backyard. The remainder of the home is excluded from childcare services. In the home, inaccessible areas are made secure by locks. At the time of the inspection, LPA observed 8 day care children present, 3 of whom were infants. Besides the Licensee, the Licensee's mother was also present at the time of the inspection helping with the care and supervision of children (cleared and associated).
LPA observed the FCCH to be clean and orderly. The bathroom, utilized for childcare, is clean and free of toxins. LPA observed bathroom cabinets to be free of hazardous items. LPA observed sharps, medication, and cleaning supplies in elevated kitchen cabinets. No children currently require medication. Licensee informed LPA that a First Aid Kit is present in the residence for the day care.
LPA observed age-appropriate toys, furnishings, and equipment throughout the main day care room. In the living room, LPA observed a wood-burning fireplace that is enclosed by glass and also has a child safety lock attached. In in the family room, an area that is inaccessible, LPA observed an additional fireplace that was covered by metal chains. Although the area if off-limits, LPA advised the Licensee that an additional barrier for the fireplace should be added. Licensee informed LPA that the fireplaces are not during day care hours. LPA observed two dogs present on site. Licensee informed LPA that children have interactions with the dogs and that their vaccines are current. (CONT. 809-C, Page 2)
