25 | On July 9, 2024, Licensing Program Analyst (LPA) Susana Martinez conducted an unannounced Case Management Inspection to follow up on an unusual incident. LPA met with site supervisor Tasha Ricketson and Director Tania Pacheco and advised them of the purpose for the inspection. LPA was provided a tour of the facility. At the time of inspection there were 18 infants in care of 7 staff.
On 6/11/2024, Tania Pacheco contacted Community Care Licensing (CCL) to self-report an incident of a child (C1) who approached the water dispenser and was able to push the safety button, resulting in hot water pouring into C1's hand.
On 6/10/24 an incident occurred at the center during transition into nap room. Director states that children have to walk through area where water dispenser is located. The water dispenser has two buttons located in the front, one of them being a safety button. C1 walked over to the water dispenser and managed to press both buttons which in turn dispensed hot water. At the time of the incident there were four staff with 12 infants. Parent of C1 works on site and was informed right away. C1 was taken to urgent care for a medical evaluation. Medical provider diagnosed injury as a first degree burn. C1 returned to the day-care facility on the third day after the incident.
To prevent any similar incidents, the facility has removed the hot water source completely. During regular morning checks, the facility also checks the water dispensers to ensure there is not hot water accessible to children. Director states that prior to the incident facility was not checking the dispensers as it was assumed that the hot water was not available as center has always had the hot water switch turned off.
LPA interviewed staff who were present at the time of incident. LPA determined that the incident was true and did occur. Based on staff interviews there were three staff present. Staff interviewed indicated children have walked over to the water dispenser before but hot water was never dispensed as it was not switched on.
