On 7/12/23, at 12:00 PM, Licensing Program Analyst (LPA) Elvin Baddley conducted an unannounced One Year Required Inspection of the abovementioned Child Care Center (CCC). LPA met with Heather Robbins, Licensee of the CCC and explained the purpose of the inspection. LPA, in the company of the Licensee toured the interior and exterior of the CCC. The center operates from 7:30 AM to 5:30 PM, Monday - Friday. The CCC is a dedicated infant center and is on the grounds of Emanuel Lutheran Church. The CCC is comprised of three classrooms (Young Infant Room, Older Infant Room and the Toddler Room) in two buildings. At the time of the inspection, 32 children are present, along with 15 teachers (cleared and associated) providing care and supervision.

LPA observed the CCC to be organized. The CCC has ventilation and heating to afford for child care. LPA observed age appropriate toys and furnishings throughout the CCC. The CCC has an abundance of cribs and cots for children in care with various levels of mobility. LPA observed changing tables on site within arms reach of sinks. The CCC restroom used by the children was observed to be clean and free of toxins.

LPA observed carbon monoxide detectors in each classroom. A detector in the Older Infant Room was tested at 2:10 PM and found to be operable. LPA observed refrigerators in each room which contains labeled and dated foods and liquids. Licensee informed LPA CCC provides snacks for children in care who are able to consume snacks. The CCC provides bottled water. Licensee informed LPA the CCC's water sources have not been tested for lead containment levels. LPA reminded the Licensee of the mandate to have CCC's water sources tested for lead containment levels by 1/1/23. LPA observed licensing required notices and forms at the entry of the CCC. Medication, sharps and cleaning solutions are secure in locations inaccessible to children in care.

The CCC electronically signs children in and out of the CCC. LPA reviewed the Sign In/Out system which contains arrival and departure times of children as well as electronic signatures of authorized representatives. .
The CCC's play areas have varied footing and are enclosed. Gates leading to the play areas are secure. LPA (CONT. 809-C, Page 2)

observed age appropriate toys/play equipment in the play areas. The toys/play equipment are free of sharp, loose or pointed objects. Licensee was reminded to replace toy and play equipment when such begins to degrade or is not in good repair. LPA observed no bodies of water were observed on site.

A sampling of staff and children's records were reviewed. The children’s records were complete and found to contain emergency contact information, immunization records and electronic logs documenting 15 minute sleep checks, among other licensing required documents. LPA reviewed Individual Sleep plans which were completed appropriately. Staff records were reviewed and current with Pediatric First Aid/CPR (EMSA Approved) and Mandated Reporter Training certifications, as well as documentation supporting qualifications and experience. Licensee informed LPA no firearms are on site.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of “medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Facility representative was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

A Type B Deficiency are being cited based on LPA's observation and interview pursuant to Title 22 of the CA Code of Regulations (refer to LIC 809-D). Licensee was provided a copy of their Appeal Rights (LIC 9058) and their signature on this form acknowledges receipt of these rights.

Licensee confirmed that there are no Registered Sex Offenders living in the facility and LPA completed (CONT. LIC 809-C, Page 3)
