Per LPM Mueller, report is amended on 5/20/2024

On 05/07/2024, at 10:00 AM, Licensing Program Analyst (LPA) Joaquin Mendez conducted an unannounced Three Year Required Inspection the abovementioned Family Child Care (FCC). LPA met with the Licensee, Lissette Katia Zarate. LPA explained the purpose of the inspection. Entrance Checklist was provided to the Licensee.

LPA, in the company of Licensee toured the interior and exterior of the FCC. LPA reviewed licensing required notices and documents posted predominately at the entry of the FCC. The FCC operates from 5:40 AM to 5:30 PM, Monday – Friday, closed on holidays. The FCC primarily uses the living room area and one (1) restroom for childcare. LPA observed 6 children in care during the inspection. LPA notes there was one (1) other adult, assistant (S1) at the time of inspection.


The rooms used for childcare are clean and organized. The rooms have age-appropriate toys and furnishings available for children in care. The FCC has a smoke detector which was tested at 11:20am and carbon monoxide detector and was tested at 11:21 in the living room hallway. Both smoke detector and carbon monoxide detector were operating at the time of inspection. The restroom used by children in care is observed safe and in sanitary operating condition.

The FCC provides snacks and lunches for the children in care. In the home, drinking water is supplied with a water container and outside drinking water is supplied with a pitcher. LPA notes each child in care has his/her own drinking cup and is labeled. Licensee provides brochure information to parents with information about lead in water supply and was verified by LPA.

Disinfectants, cleaning solutions, and medications in FCC are in the garage which is inaccessible to children in care. Sharps are in a locked upper cabinet in the kitchen and are inaccessible to children in care with a gate. LPA observed the required fire extinguisher (2A10BC) in the home and was serviced on 1/23/2024. LPA reminded Licensee of the responsibility to service or purchase a regulation fire extinguisher annually.

The FCC's outdoor playground areas are enclosed by fencing. The fence's entry/exit gates are secured and the footing in the areas is made up of various surfaces such as Synthetic grass and paved concrete. Toys, furnishings and play equipment in the outdoor area is age appropriate. The toys, furnishing and play equipment are in suitable condition and free of sharp, loose or pointed objects. The play structures in the playground areas have cushioning materials to absorb falls on outdoor structures. Licensee was advised to provide shade for children in care as the weather permits for hotter weather. Licensee is reminded to replace toys, furnishing and play equipment when they degrade and are not in good order. No bodies of water were observed on site.

LPA’s record review revealed Preventative Health training was complete on 2/21/2020. Further, Licensee completed Mandated Reporter training and expires 9/12/2024 and Pediatric CPR/First Aid (EMSA approved) and expires 5/09/2026. Continue 809C pg3

LPAs reminded Director of obligation to maintain current training and certifications. Applicant does not have liability insurance for the license.

Licensee owns the home:The Licensee provided proof of control of property.


A sampling of staff and children records were reviewed. The children’s records were complete and found to contain emergency contact information as well as immunization records, amongst other required licensing documents. Staff records were reviewed and found to contain pediatric CPR and First Aid certifications (EMSA approved) and were documented to expire 3/09/2026, Mandated Report certifications expire 09/12/2024, amongst other required licensing documents.

Licensee stated she is not providing IMS.Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. LPA also informed Licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/, and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

On this date, 5/07/2024, the California Attorney General - Megan’s Law website was searched for information on sex offenders required to register with local law enforcement under California's Megan's Law. No registered sex offenders were found at the facility addresses. Under state law, some registered sex offenders are not subject to public; therefore, they may not have been included in this search. However, the Department conducts a monthly cross reference of each address on record for all registered sex offenders against all CCLD facility addresses pursuant to information shared by California DOJ.
Licensee, informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California.

A notice of site visit was given to Licensee, and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days.

Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms.

Licensee was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.
