On 11/22/24, at 9:10 AM, Licensing Program Analysts (LPAs) Elvin Baddley and Fernando Hernandez conducted an unannounced Annual/Random Inspection and met with Sierra Labossie-Duarte, Licensee of the abovementioned Family Child Care Home (FCCH). LPAs informed Licensee of the nature and purpose of the inspection. LPAs, in the company of the Licensee, toured the exterior and interior of the FCCH. The FCCH is a three bedroom, two bathroom single story dwelling. The FCCH's family room, living room, guest rooms (2), hallway bathroom and backyard are be used for child care services, while the remainder of the home is excluded from care. LPAs observed child safety gates at the entry/exits of the kitchen. At the time of the inspection, LPAs observed two children in care (biological).

The FCCH has spacing and ventilation for children in care. LPAs observed two fireplaces on site which were screened by metal mesh. LPAs observed the bathroom used by children in care to be clean and free of toxins. Medication in the FCCH's is stored on "pantry" shelf in an elevated location. Sharps and cleaning compounds in the FCCH's are observed in the kitchen which is an area excluded from childcare services. LPA observed the FCCH to be orderly.

LPAs observed required licensing documents and notices posted on the wall of the FCCH in the family room. LPAs observed a fire extinguisher (2A10BC) in the FCCH's kitchen which was serviced on 9/25/24. LPAs reminded Licensee of the responsibility to service or purchase a regulation fire extinguisher annually. The home has numerous smoke and carbon monoxide detectors. A smoke detector in the FCCH's family room was tested at 9:37 AM and found to be operable, while a carbon monoxide detector in the family room was tested at 9:36 AM and found to be operable. The FCCH has age appropriate toys and play equipment available to children in care. The FCCH has operable telephone services.

The FCCH's backyard is enclosed by a combination of wooden and cinder block fencing. Footing in the area is varied. The backyard fences entry/gates (2) are secured. (CONT. LIC 809-C, Page 2)

The backyard is partitioned in a fashion which an area/space for gardening is separated from the area used by children in care. The backyard area has age appropriate toys and play equipment. LPAs observed no bodies of water on site.

LPAs reviewed children, staff and facility records. Children's records are current, complete and possessed emergency contact information, immunization records and sleep logs, among other required licensing forms and documents. The Licensee's records are current and complete with Pedantic CPR/First Aid certifications (EMSA approved) expiring on 5/13/25, and Mandated Reporter training certification expiring on 5/29/25. LPAs reviewed the FCCH's Fire Drill log which noted the last fire drill occurred on 9/20/24. The FCCH has a facility roster which contains the names and contact information of children in care. The FCCH has a dog on site. LPAs reviewed the vaccination record for the dog which are current. The Licensee informed LPAs no firearm or ammunition are stored on site.

The Licensee does not provide Incidental Medical Services (IMS). IMS policy was discussed. For IMS information see PIN 22-02CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. LPAs discussed the safe sleep regulations with Licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPAs also informed Licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Licensee was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to (CONT. initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

During the exit interview, the Licensee confirmed that there are no Registered Sex Offenders living in the facility and LPAs completed the RSO profile in FAS on 11/22/24.

No deficiencies site during todays inspection.

A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Licensee Sierra Labossie-Duarte.
