32 | Based on LPA's observations, interviews, review of documentation, and evidence gathered, the preponderance of evidence standard has been met. Therefore, the allegation that the facility is operating over capacity is determined to be SUBSTANTIATED.
As a result, one Type A deficiency is being cited today under California Code of Regulations, Title 22, Division 12 on the attached LIC 9099D. LPAs Billones and Reyes informed the Licensee Yesenia Mendoza that this report dated 06/20/2025 documents one (1) Type A citation which shall be posted for 30 days as there is immediate risk to the health, safety, or personal rights of children in care.
LPAs Billones and Reyes informed the Licensee to provide a copy of the licensing report dated 06/20/2025 that documents any Type A citations to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
The Notice of Site Visit (LIC 9213) was provided. The Notice of Site Visit must remain posted in a visible location for 30 consecutive days. Failure to comply with posting requirements will result in a $100.00 civil penalty.
An exit interview was conducted, appeal rights were provided, and report was reviewed with the Licensee, Yesenia Mendoza. |
