On 08/07/2023, Licensing Program Managers (LPMs) Maria Mueller and Ana Tolentino along with Licensing Program Analysts (LPAs) Francisca Velazquez, Martina Jimenez and German Negrete met with Licensee, Jessica Garcia for an Informal Conference. The purpose of this Informal Conference is to review and discuss the following deficiencies:
  • CCR 102425 - Infant Safe Sleep - Child #6 was observed strapped in car seat during inspection.
  • CCR 102370(d) - Criminal Record Clearance - Assistant, Damaris Alexia Esparanaza Sosa is not fingerprint cleared and associated to the facility.
  • CCR 102416.5(d)(2) - Staffing and Ratios - 24 children were present during inspection, with 6 children being infants.
  • CCR 102417(g)(1) - Operation of a Child Care Home - Expired fire extinguisher since 2/28/2023
  • CCR 102418(g) - Immunizations - All 24 children present did not have immunization records readily available for review.
  • CCR 102421(b) - Child's Records - 18 children present did not have child files reality available for review.
  • CCR 102417(g)(7) - Operation of a Child Care Home - 18 children present did not have emergency information cards readily available for review.
  • CCR 102417(g)(8) - Operation of a Child Care Home - Licensee did not have current and updated facility roster.
  • CCR 102419(d)(1) - Admission Procedures and Parental and Authorized Representative's Rights - 18 children present did not have Notification of Parent Rights LIC995A readily available for review.
  • CCR 102417(m)(3) - Operation of a Child Care Home - 18 children did not have signed affidavit's.
  • CCR 102425(c) - Infant Safe Sleep - 2 infants did not have Individual Infant Sleeping Plan LIC9227 on file to review.
  • CCR 102425(j)(2)(D) - Infant Safe Sleep- 3 infants did not have infant sleep charts documenting 15 minute checks. CONT 809-C

In response to these discussion, licensee has agreed to the following:
  • Licensee shall ensure that staffing ratio will be met at all times. Licensee will not exceed licensed capacity at any time.
  • Licensee shall ensure children are provided safe and healthful environment.
  • Licensee shall operate in compliance of Title 22, Division 12 Family Chid Care Home (FCCH) Regulations at all times.
  • Licensee shall submit a hand written plan of correction by 08/11/2023 detailing how the FCCH will comply with the regulations at all times.
  • Licensee will be referred to Technical Support Program for training to keep the FCCH in compliance.
  • Licensee was provided Smart Horizon flyer to take classes to help in her child care operation. Licensee shall complete training in Commitment to Professionalism- Ethic and Ethical Behavior, Effective Program Operation- Environmental Safety and Staffing Child Care Programs and Safe and Healthy Environment- Infant and Toddler Health and Safety.
  • Licensee shall attend in person orientation on 08/17/2023 in San Luis Obispo, CA.
  • Licensee was provided with CCLD website training videos https://ccld.childcarevideos.org/
  • Increased unannounced visits to monitor compliance for 2 year period to be conducted by CCLD.

Upon receipt of the final report, provide copies of this licensing report to each parent/guardian of enrolled children and to parents/guardian of newly enrolled children during the next 12 months. Acknowledgement of Receipt LIC9224 form shall be used for this purpose. LIC9224 after completed shall be maintained in each child's file. (LIC9224 form was given to Licensee at the conference today).
