25 | On December 6, 2023, Licensing Program Analyst (LPA) Susana Martinez conducted an unannounced Case Management inspection. LPA met with Office Coordinator Artisia Carlon and advised her the purpose of the inspection. LPA was provided a tour of the facility. At the time of inspection there were 21 children in care of 4 staff.
On 9/12/23, Associate Teacher Juana Zarrate contacted Community Care Licensing (CCL) to self-report an incident involving a child (C1) who was left unattended in the outdoor play area on 9/8/23.
LPA Martinez interviews Associate Teacher Juana who states a parent came into the facility and talked for a couple seconds. While teacher and parent were talking, the door was left opened. Once the parent left, teacher closed the room door. According to teacher Juana, a couple seconds later a teacher aid working on the yard knocked on the door. The teacher aid pointed out one of the center's children sitting on the outdoor play structure. Child was brought back into the classroom. Teacher Juana estimates this incident happened within one minute. Child was not found in distress.
To prevent any similar incidents, teacher Juana states they have implemented adding a baby gate at the entrance of the door while the door is open. This will prevent children from leaving the room. Teacher Juana also states teachers are constantly conducting head counts to keep track.
Given the incident was observed by staff who took appropriate action by reporting the incident to Licensing and implementing new protocols for the room, no deficiencies are being issued as a result of this incident.
