This is a change of location, previous facility number 426215931.

Due to COVID-19 pandemic, LPA asked the pre-screening questions prior to inspection. Licensee's responses indicate there was no COVID-19 exposure on site.

On 10/11/22, Licensing Program Analyst (LPA) Francisca Velazquez conducted an announced change of location, Pre-licensing inspection for a Large Family Child Care Home (FCCH) license. LPA met with Yesenia Michel, Applicant, and discussed the purpose of today’s visit. LPA and Applicant together toured the inside and outside of the home. There were no children in care at the time of the inspection.

Licensee requested a change of location for a large family childcare license. The home has five (5) bedrooms, two (2) bathrooms, living room, daycare room, dining room, kitchen, garage and outdoor yard. The licensee will use the daycare room, one bedroom for napping purposes, kitchen, dining room, bathroom, and outdoor yard for childcare services.

LPA observed a small baby gate located on the first step of the stairway making the entire second floor inaccessible to children in care. LPA observed bedrooms on the first floor are locked and inaccessible to children in care. LPA observed the garage is located in the front of the FCCH and is not accessible to children in care. LPA observed that part of the outdoor yard is made inaccessible by means of a wood fence.

LPA observed that the FCCH is clean and orderly. In addition, there is plenty of ventilation for the children in care. CONT 809-C
