On 5/3/2024, at 10:00 AM, Licensing Program Analyst (LPA) Gigi Reyes conducted an announced Pre-licensing Inspection of the above-mentioned home and met with Applicant. Adriana Castillo, LPA informed Applicant of the nature and purpose of the inspection. Applicant plans to operate the Family Child Care Home (FCCH) from Monday to Saturday, 5:30 AM- 6:00 PM. Applicant plans to care for children 0 year to 12 years of age. No children was observed at the time of the inspection.

LPA, in the company of Applicant, toured the interior and exterior of the home. The residential home is a two story home located in gated community, Mariposa Town homes. The home consists of 3 bedrooms and 1 and ½ baths. The home’s living room, dining area and patio will be used for childcare services, while the home’s second floor where bedrooms are located will be inaccessible to children. The top and bottom of the staircase are barricaded making it inaccessible. The kitchen and garage are also kept inaccessible to day care children. Door leading to garage has a door safety lock and kitchen is barricaded by a baby gate.



LPA observed the home to be clean and orderly. The home has spaced and ventilation for children in care. The home has no fireplace; however, the home has a wall heater located upstairs with a vent that extends to the living room.

The bathroom to be used by children in care is observed to be clean and free of toxins. LPA observed cleaning solutions secured on top of the kitchen cabinet. Sharps are observed on top of the refrigerator, while medication in the home is kept in the bedroom The aforementioned items will be inaccessible to children.

LPA observed a fire extinguisher (2A10BC) in the home which was purchased on 2/9/2024. LPA reminded Applicant of the responsibility to service or purchase a regulation fire extinguisher annually. The home has functioning smoke and carbon monoxide detectors

The patio is enclosed with 4- foot vinyl fence installed horizontally, with 6-inch gap in between the panels. LPA reminded Applicant to ensure direct supervisionwhile children are playing in the patio area. No bodies of water were observed on site.


LPA's record review revealed Applicant's Mandated Reporter training was completed on 1/17/2024 and Pediatric CPR/First Aid (EMSA approved) training was completed on 12/9/2023 Applicant completed Preventative Health Training on 11/18/2023, and the CCLD Orientation on 12/13/2023. LPA reminded Applicant of obligation to maintain current training and certifications. Because the Applicant rents/leases the home, proof of landlord notification is required.

The LPA observed the Property Owner/Landlord Notification form (LIC 9151) that the applicant confirms was provided to the property owner/landlord.
The applicant has not obtained a signed Property Owner/Landlord Consent form (LIC9149). Without this consent, the applicant understands that, once licensed, they can operate with a maximum capacity of 6 children. If property owner/landlord consent is obtained in the future, the applicant is advised that a new Application for a Family Child Care Home License (LIC 279) must be submitted with a change of capacity fee of $25, to increase the capacity and provide care to 8 children.

LPA reviewed with Applicant the LIC 311D, Forms/Records To Keep In Your Family Child Care Homes, children’s forms/records, facility forms/records, and information to be posted.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. LPA also informed Applicant of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/, and recommended they register all infant devices with the CPSC to be notified of any recalls of infant devices or their purchased equipment.

Applicant was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

On this date, 5/3/2024, the California Attorney General - Megan’s Law website was searched for information on sex offenders required to register with local law enforcement under California's Megan's Law. No registered sex offenders were found at the facility address. Under state law, some registered sex offenders are not subject to public disclosure; therefore, they may not have been included in this search. However, the Department conducts a monthly cross reference of each address on record for all registered sex offenders against all CCLD facility addresses pursuant to information shared by California DOJ.

Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platform.

Applicant was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California.

Notice of Site Visit was issued and must remain posted for 30 days.

The issuance of license is pending completion of setting up the day care are with toys, equipment, and books as well as setting up the playpen and cots.
