On December 30, 2025 at 9:00 AM, Licensing Program Analyst (LPA) Bill Billones conducted an announced change of location, Pre-licensing inspection with an Applicant requested change of capacity to a Large Family Child Care Home (FCCH) license. LPA met with Applicant Ana Murguia Alvarado and discussed the purpose of today’s inspection. LPA and Applicant toured the inside and outside of the home. There were no children in care at the time of the inspection and no other adults present. Applicant stated childcare will be provided Monday through Friday from 6:00 AM to 6:00 PM, and Saturday from 6:00 AM to 2:00 PM serving ages 0 to 13 years old.

The residence is a two-story 5 bedroom, 3 bathroom home with an attached garage. The FCCH will operate in a day care room, 1 bedroom, and 1 bathroom on the first level as listed in the facility sketch. Day care children will also have access to the living room, dining room, kitchen, and backyard. LPA observed stairs are gated and the entire second level is off limits. LPA observed the home to be clean and orderly with adequate spacing and ventilation. Age-appropriate toys and napping equipment were observed in the day care room and bedroom. The bathroom was observed to be clean, sanitary, and free of hazardous items. LPA observed sharps and cleaning supplies are stored in an elevated shelf in the kitchen. Lower kitchen cabinets are kept secured by child locks. Personal medications were observed in an elevated shelf in the hallway closet. LPA observed off-limit rooms are secured by child proof door knob covers. The heater closet door was not secured. Applicant was reminded to store hazardous items out of reach from children in care and that in order for areas to be inaccessible they must be secured while day care children are present.

• Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.

• Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

A regulation fire extinguisher (3A40BC) was observed with a purchase receipt dated 09/18/2025. Applicant is reminded to service or purchase the fire extinguisher yearly. LPA had Applicant test dual smoke and carbon monoxide detector in the living room, which was found operational at 9:27 AM. All adults residing in the home have background clearance. Applicant states one dog lives in the home and confirmed vaccines are up to date.

Applicant is current with immunization required per SB792. Applicant's Pediatric First Aid/CPR certificate expires on 10/14/2027. Applicant's Mandated Reporter training certificate is valid until 09/15/2027. Preventative Health and Safety and Nutrition Training was completed on 01/21/2024. Childcare orientation was completed on 12/21/2023.

Applicant was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

The Applicant provided proof of control of property.

Because the Applicant rents/leases the home, proof of landlord notification is required. The LPA observed the Property Owner/Landlord Notification form (LIC 9151) that the Applicant confirms was provided to the property owner/landlord. The Applicant obtained a signed Property Owner/Landlord Consent form (LIC 9149).

LPA discussed the safe sleep regulations with Applicant discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep, as an additional resource. LPA also informed Applicant of the importance of checking for and removing recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/, and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

On this date 12/30/2025, the California Attorney General - Megan’s Law website was searched for information on sex offenders required to register with local law enforcement under California's Megan's Law. No registered sex offenders were found at the facility addresses. Under state law, some registered sex offenders are not subject to public disclosure; therefore, they may not have been included in this search. However, the Department conducts a monthly cross reference of each address on record for all registered sex offenders against all CCLD facility addresses pursuant to information shared by California DOJ.

Applicant was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters, and other important information communication platforms.

• All adults living and working in the home shall be made aware of the Department’s inspection authority.In the absence of the Licensee, a qualified adult must be present supervising the children. A qualified adult is an individual who has a valid and current Pediatric First Aid/CPR certification, TB clearance, immunizations, and a valid criminal record clearance associated to the facility license.
• A current roster of children enrolled must be available and maintained for a period of three years, even after children no longer attend the facility.
• Reporting requirements: any unusual incidents or injuries must be reported to the Department within 24 hours, and a written report (LIC624B) must be submitted within seven (7) calendar days to the Department.
• Smoking is prohibited in a family child care home.
• Applicant was made aware of the responsibility to know the regulations for Family Child Care Home which can be accessed on-line at www.ccld.ca.gov.
• Applicant was provided information from PIN 20-24-CCP - Recently Approved Safe Sleep Regulations In Effect and LIC 9227 (Infant Sleeping Plan).
• Applicant was informed baby walkers, jumpers, bouncers, exersaucers, or any similar article are not permitted on the premises during day care hours.

The home meets Title 22 Division 12 requirements for a Large Family Child Care Home License with a maximum capacity of fourteen (14) children pending the following corrections:

· Applicant was advised to secure the heater closet door.

· Applicant was advised to remove or barricade the backyard tree stump with sharp branches.

License is pending Licensing Program Manager Approval. Once licensed, the Applicant is required to comply with the terms and limitations stated on the license.

A notice of site visit was given to Applicant and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days.
