25 | Licensing Program Analyst (LPA) Anna Morales conducted a Case Management inspection to follow up on an Unusual Incident that was reported by the facility to Licensing on 1/30/24. LPA was greeted by Head Teacher Joanna Gill and Whitney Lukanc, Early Learning Program Specialist. LPA conducted interviews with staff.
During the course of this investigation, LPA was informed that Site Director Silvia Bejarano has been absent from this site since October 2023. Whitney Lukanc informed LPA that they have qualified staff present in her absence. Whitney Lukanc understands that a Site Director shall be on the premises during the hours the center is in operation.
Based on interviews and evidence gathered, it was determined that on 1/30/24, approximately at 12:10pm(during transition from out door time to indoor time), a Preschool aged child was left unattended outside on the playground for approximately for five minutes. This poses an immediate risk to the health, safety, and personal rights to children in care.
A TYPE B and one TYPE A citation was issued at today's visit. For the TYPE A citation, a copy of the Facility Evaluation Report LIC809/LIC809D, has to be posted on the wall and a copy to be given to all parents of currently and newly enrolled children for the next 12 months. In addition, copy of LIC9224 Statement Acknowledging Receipt of Licensing Reports need to be signed and kept in child files. An exit interview was conducted, and Plan of Corrections were reviewed.
A NOTICE OF SITE VISIT WAS ISSUED AND MUST REMAIN POSTED FOR 30 DAYS. |
