25 | Licensing Program Analyst (LPAs) Manuel Monter and Mita Partoza conducted an unannounced annual inspection visit, and met with staff S1, Lolita Ignacio. S1 contacted facility ADM, and ADM stated she would be able to be at the facility by 11am. During the visit, LPA observed 2 residents and 2 staff.
When LPA's entered the home, Staff S1 was updating the Medication Administration Record, Staff S2 is the 1 on 1 care giver for R4 and was accompanying R4 in his/her bedroom. Staff S3 was in his/her bedroom because it is his/her day off.
LPA toured the facility inside out with S1 which included the Living room, kitchen, dining room, 3 restrooms and 5 residents bedrooms. The staff area of the facility was also inspected. The front yard and backyard were inspected. There was no obstruction to block the walkways.
While touring the facility Laundry room, LPA's observed a container of detergent accessible to residents in care. (Photographs were taken.)
While touring facility bathroom adjacent to bedroom #6, LPA was informed by staff S1 and S3, that the toilet is clogged. S1 stated the toilet has been clogged since Monday. S3 stated the toilet has been clogged since Wednesday. ADM stated the plumber is scheduled to come to the facility on 5/25/2024. ADM stated she would send LPA a copy of the receipt.
Two-day perishable food supplies and seven day nonperishable food supplies were observed. LPA observed the medication storage area, knives storage area, and cleaning product storage area as locked and inaccessible to residents in care. Room temperature was at 70 degrees F, and hot water temperature was measured to range from 111-112 degrees F in resident bathrooms.
