13 | Licensing Program Analysts (LPAs) Marilou Monico and Yareli Ramirez conducted an unannounced follow up complaint inspection. LPAs met with Site Director, Cuc Tran. LPA toured the facility, interviewed staff, and census was taken. During the investigation process, staff and parents were interviewed and pertinent documents were obtained.
Based on the information gathered from interviews as well as supporting evidence, despite staff supervision and efforts to intervene and prevent physical contact, a child still sustained injuries from another child. Although the allegations may have happened or is valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are UNSUBSTANTIATED.
There were no deficiencies cited.
Exit interview conducted and report was reviewed with Site Director, Cuc Tran. Copy of appeal rights was provided to Site Director.
A Notice of Site Visit was issued and must remain posted for 30 days. |
