On 08/21/2026, Licensing Program Analysts (LPAs), Farida Raja and Sara Mulugeta conducted an unannounced complaint visit to deliver investigation findings for the above allegation. LPAs met with Director, Lynda Nguyen and explained the purpose of today's visit.

During today's inspection, LPAs toured the facility and observed 5 staff and 36 children in the preschool program. Facility is operating within the ratio and capacity requirements of the license.

The complaint alleges that staff handled child in a rough manner resulting in an injury. During the course of the investigation, LPA interviewed staff including director, assistant director and children and obtained relevant records including the police report.

Based on interviews, during play, child was stuck under a table. Staff intervened and got child out. Based on statements, staff gently helped child out from under the table. Child sustained an injury on the chin. At this time, there is not sufficient evidence to prove if the injury sustained was from child being stuck under the table or from staff's handling of the child to remove them.

It is concluded that, although the allegation listed above may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur. The allegation is thus UNSUBSTANTIATED.

As a result of today’s inspection no deficiencies were cited. Exit interview conducted and report was reviewed with Director, Lynda Nguyen. Appeal rights provided.
