Licensing Program Analyst Jennifer “Jen” Beehler conducted an unannounced Complaint Investigation. Upon arrival, LPA was greeted by Director Leah Kidger and provided access to the facility. LPA stated the reason for the visit was to deliver findings.

On 05/06/2025, LPA conducted an unannounced initial complaint inspection to open the investigation. LPA conducted confidential interviews, observed the facility and collected relevant documentation. LPA conducted a records review and interviewed parents throughout this investigation.

During this investigation, LPA determined that C#1 and C#2 were provided food by the facility that they were either intolerant or allergic to on 04/18/2025 and 04/25/2025.

LPA received photographic evidence that the facility has not been providing fruit and/or vegetable with every meal or snack. Based on interviews conducted, fruit that is planned and posted on the menu has been missing from snacks.

Parents stated they have witnessed children in care using the restroom without supervision. LPA has observed the children using the restroom without visual supervision. Staff interviews revealed that teachers have left the bathroom during restroom times to address other issues in classrooms.

Based on LPA’s observation, interviews conducted, and evidence gathered during the investigation process, the preponderance of evidence standard has been met, therefore the above allegations are SUBSTANTIATED.

Due to today’s investigation, One Type A and One Type B Deficiencies are cited along with civil penalties assessed. More information on the attached LIC9099D and LIC421FC. Lack of Supervision was cited on 06/17/2025. Director provided a written plan of corrections to LPA on 06/18/2025 to address the deficiency. Exit interview conducted with Director Leah Kidger, report reviewed and provided along with appeal rights.

Director to develop and implement a written plan that addresses the procedure for ensuring the dietary restrictions, allergies and food intolerant needs of children in care are consistently addressed. Director to train staff on these new procedures and provide LPA with the written procedure along with a sign in sheet on 06/20/2025 verifying training attendance.
