Licensing Program Analysts (LPAs) Mel Matos & Jennifer Beehler met with Margarita "Maggie" Corral, Director, for an unannounced case management investigation in response to an Unusual Incident that the Facility self reported to the Department.

LPAs discussed the incident with Maggie, reviewed Facility documents, and interviewed staff during today's investigation.

Maggie states that corporate is conducting an internal investigation regarding the Unusual Incident and is not sure when the investigation will be completed.

Further investigation is required. A follow up investigation will be conducted at a later date.

Exit interview conducted and the report was reviewed with the Director, Maggie Corral. No deficiencies issued during today's investigation. Notice of site visit issued and must remain posted for 30 days.
