25 | Licensing Program Analyst (LPA) Anna Morales conducted a Case Management visit in response to an Unusual incident that was self reported by the facility to Licensing on 3/10/25. LPA met with Director Melody Eslamain Maher. LPA toured the facility, interviewed staff, reviewed and obtained documents.
Based on available evidence and interviews, it was determined that on 3/7/25, Staff (S1) left 10 children playing in the playground with no supervision at approximately at 4:55pm for approximately 30 seconds to two minutes. According to the Child Supervision Record (CSR) there were eleven(11) Preschool students. This poses an immediate risk to the health, safety, and personal rights to children in care.
LPA informed director that this report dated on 03/14/2025 documents One Type A citation which shall be posted for 30 consecutive days as there is immediate risk to the health, safety, or personal rights of children in care.
Also, LPA informed the director to provide a copy of this licensing report dated on 03/14/2025 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. The citation warrants an immediate civil penalty of $1,000.00 (assessed for repeating the same violation within 12 months). Please see LIC421IM.
Exit interview was conducted with Director Melody Maher, and Plan of Corrections were discussed. A notice of site visit has been issued and must remain posted for 30 days. Appeal Rights Given.
