25 | Licensing Program Analyst (LPA), Shine Yu, conducted an unannounced Annual Inspection 02:05 pm and was greeted and met with the Licensee, Garcia Tish.
LPA observed the required posted materials, including the Facility License, Emergency Disaster Plan (LIC610), and Parents' Rights (PUB 393). Facility's operating days and hours are Monday to Friday 8:00 AM to 5:00 PM. LPA observed a current Children's roster. The last fire drill was conducted on 04/18/2025. Present are licensee, 1 helper(licensee's daughter) and 10 children, all over 2 years old. LPA observed all children are taking nap time during the inspection. There are 3 Shih Tzu dogs.
LPA reviewed a random selection of 5 children's files and observed that notification of parent's rights forms, immunization records forms, consents for emergency medical treatment forms, and Identification forms are in each file.
LPA also reviewed the staff files for the following records: Employee Rights (LIC9052), Statement Acknowledging Requirement to report Child Abuse (LIC9108), and Immunization Record showing immunity to measles (MMR), pertussis (Tdap), and influenza. Licensee's pediatric 1st Aid and CPR certificate, expires January 18, 2027. Licensee and her daughter's mandated reporter training certificate expired on June, 2027.
Licensee accompanied with LPA to tour the facility. Off limits areas inside the home are section A, office room and bed room in section B. LPA observed the fire extinguisher in the kitchen, the smoke and carbon monoxide detector are in living room. The bathroom is clean and organized. There are age-appropriate toys in the living room and play yard for children. Licensee stated there is no firearm in the facility. There is no stairs in the facility. There is no body of water observed outside of play yard. The climbing structure is surround with resilient materials and the play yard is covered with artificial turf. The outdoor play area is enclosed with appropriate fence and safe for children.
