On Wednesday, August 24, 2022 9:30 am, Licensing Program Analyst (LPA) Manel Estoesta conducted an unannounced Required 1 Year Visit. LPA met with the Executive Director Meghan Galazzo, the Center Director Nicole Baker and explained the purpose of today's visit. Present on this visit were 37 staff and 132 children in care. Facility's operating days and hours are Monday to Friday 7:30 am to 6:00 pm

LPA toured the facility to conduct a Health and Safety Inspection with the Center Director. The Facility has an infant program with a license # 434 404 322. The Preschool Program Licensed Rooms are Home Base Room 9 to 17 (South Side) and Home Base Room I to R (North Side). Currently, the Preschool Program is only using the Home Base Room 9 to 14, 17, N and O.
Facility’s License, Parents’ Rights Poster, Personal Rights, Activity Schedules and Waivers were observed to be posted. Facility was observed to follow teacher to children ratio requirement. Children were engaged in various activities under the visual supervision of the teachers.

The classrooms, restrooms, kitchen, storage room, and office area were inspected. Disinfectants, cleaning solutions, and other items that are dangerous to children were stored inaccessible to children. Cabinets, drawers, and rooms used for storage were locked. Furniture and equipment such as mats, cots, tables, and chairs were age appropriate and were in good condition, free of sharp, loose, or pointed parts. Restrooms for children's use were observed to be in safe, sanitary, and functioning condition. Floors were clean and free from tripping hazard.
Outdoor activity space is fenced and was inspected. The play equipment was maintained in a safe condition and free of hazards. There were no bodies of water observed. Areas around and under high climbing equipment and slides were cushioned with material that absorbs falls. There were shaded rest areas for children. Drinking water are arranged to be readily available to children during indoor and outdoor activities.

LPA reviewed the following;
1. Fire Drill were conducted and last drill conducted 07/20/2022.
2. Earthquake Drill were done in 04/28/2022.
3. "My Bright Day" Software Application for Facility Operation such as but not limited to Children's Sign in and Out, Napping Log, Activity Log and Parent Teacher Communication.

Facility files were reviewed. Child sign in and out procedures, logs were reviewed including the "paper" or manual copy. LPA also obtained copies of the Facility's updated Personnel Report, Child Care Facility Roster and Children's Sign in and Out.

Staff files were reviewed. Children Files were reviewed. Each child’s file contains but not limited to Individual Care Plan, Personal Care Plan, Incident reports and Allergy Health Plan.

LPA discussed the PIN 22-20-CCP, Guardian User Information and COVID19 19 Child Care Guidance from the Public Health.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Directors were reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

In the areas that were evaluated, no deficiencies were observed at the time of the visit.

A notice of site visit was given and must remain posted for 30 days.

Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
