On 08/14/2026, Licensing Program Analyst (LPA), Farida Raja, conducted an unannounced annual inspection. LPA met with Licensee, Yolanda Lepe and explained the nature of today’s inspection. Present in the home were licensee, assistant and seven children including two infants and 5 preschoolers. Two additional preschoolers arrived during the inspection. Licensee is operating within the ratio and capacity requirements of the license. Days and hours of operation are Monday to Friday, 5:00 AM to 5:30 PM. LPA observed required posted materials in the play room and kitchen of the home. LPA reminded Licensee that postings need to be placed where they are visible to parents during drop off and pick up. Per Licensee, two adults and no minor children reside in the home.

LPA toured the indoor and outdoor areas of the home during today's inspection. Licensee has a working telephone in the home. LPA observed sufficient materials, toys, and play equipment for the day care children. LPA observed a wall mounted air conditioner and free standing fans in the play area of the home and license stated that she uses heaters during cold weather for comfort of children. The home is single story with three bedrooms and 2 bathrooms. LPA reviewed the current facility sketch and confirmed that the daycare room plus bathroom, living room, kitchen, one bedroom and one bathroom are used for providing care and are accessible to children. Off limit areas inside the home: master bedroom, master bathroom, one bedroom, and attached garage. LPA observed a fireplace with a glass cover in the living room area.

Backyard is fenced. The outdoor space and play equipment were observed to be age appropriate. LPA observed shaded areas provided by pergola and canopies. LPA observed artificial grass under high climbing structures to absorb falls. LPA reminded Licensee to follow manufacturers age recommendations for the outdoor play structures. LPA observed that infants have a separate gated area outdoors.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

Areas off limits to children are made inaccessible by a gate on the left side (when facing the home) and locked shed and toys on the right side. In the outdoor area, LPA observed a two-tiered water table with some water and toys. No children were present outdoors and were all under supervision by licensee and assistant indoors. LPA reminded Licensee about the Department's zero tolerance policy regarding bodies of water. Licensee emptied the water table and understands that all bodies of water including water tables, wading pools and similar items need to be emptied after each use. LPA also advised Licensee to immediately dispose any water that collects in the basin under the potable sink in the outdoor area. LPA observed wooden shelves in the outdoor area that show wear and tear and LPA informed Licensee that children could be exposed to splinters which could pose a hazard. License stated that she will cover the exposed surfaces or replace them. LPA also observed that the poles of the canopy were not anchored to the ground and were tilting. The poles were observed to be tied to the pergola and fence in some places but could still pose a risk. Licensee stated that she will either anchor the canopy poles or replace with a sun sail.

Off limit areas outside the home: locked storage shed and gated left side section. LPA reminded licensee to inspect outdoor items for items in disrepair prior to allowing access to children. All items that are rusted, broken and have sharp edges need to be made inaccessible to children in care.

LPA observed a fenced air conditioning compressor in the outdoor area. The bathroom used by children was in operating condition. Toilets and faucet are clean and operable. The shower area is free of hazards. LPA reminded Licensee that toothpaste, shampoos, air fresheners, razors, mouthwash, perfumes and similar items need to be placed inaccessible to children in care.

LPA observed a 3A40BC fire extinguisher near the entrance and receipt shows date of purchase as 08/12/2025. LPA reminded Licensee that fire extinguishers need to be serviced annually and service tag or receipt of purchase need to be maintained for review. The smoke/carbon monoxide detector in the daycare room was not functional when tested. Licensee replaced the battery and LPA confirmed that the smoke/carbon monoxide detector was functional. Licensee states that she does not have any weapons in the home. Licensee states that she has some chickens in the backyard area which is off limits to children in care. Cleaning products were observed to be placed in a child locked kitchen cabinet off limits to children in care. Knives were observed to be placed in high kitchen cabinet. Licensee understands that smoking is prohibited in the home.

Licensee was reminded to notify the Department before making any alterations or additions to the home or grounds and when any change from an area of the family child care home previously identified as off limits is changed to an area where care and supervision will be provided to children in care.

Filtered drinking water is readily available for children in the home via water pitcher and plastic cups. Licensee states that she provides meals including breakfast and lunch and PM snack. Licensee understands that any food brought from home shall be labeled with each child's name and properly stored or refrigerated. LPA discussed PIN 26-08-CCP New Guidance for providing milk in child care setting and provided a copy to Licensee.

LPA reviewed a current children's roster and fire drill log during today's inspection. Fire drill was last conducted on 05/06/2026 which is within the 6-month requirement for facilities. LPA obtained copy of children's roster.

LPA reviewed a sample of five children’s files and observed files were complete with emergency information as required. Licensee states that she carries daycare insurance and LPA observed a current policy. LPA reviewed two infant's files and the files were complete with the required forms including 15 minute sleep checks. Licensee is not providing care to children under 12 months of age at this time.

LPA discussed the safe sleep regulations with licensee, and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

LPA reminded licensee that if an infant is sleeping in a separate room from where the provider is stationed the door to the room the infant is sleeping in shall remain open at all times. The provider shall be able to visually observe the infant without moving the door. Mattress shall be firm and covered with a fitted sheet that is appropriate to the mattress size, fits tightly on the mattress and overlaps the underside of the mattress so it cannot be dislodged.

There shall be no objects hanging above or attached to the side of the crib or play yard. If an infant falls asleep before being placed in a crib like in a baby swing, the provider shall move the infant to a crib/play yard as soon as possible.

LPA reviewed Licensee and Assistant's files for the required forms. A review of staff records indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. Licensee and staff have current Pediatric CPR/First Aid certification with expiration dates of 03/06/2027 and 02/20/2027 respectively. Licensee and assistant have proof of immunization in file for measles, pertussis and statement declining the influenza vaccine. Assistant was observed to have Mandated Reporter Training that expired on 06/17/2026 but Licensee's Mandated Reporter Training has expired on 06/17/2026. LPA reminded Licensee that the Pediatric CPR/First Aid and Mandated Reporter training needs to be renewed every two years.

Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Licensee stated that the form of discipline is to have the child sit and calm down if they are fighting or upset. Licensee understands that children's personal rights should not be violated; including no corporal punishment. Children have personal rights that cannot be waived or abridged regardless of consent or authorization from the child’s authorized representative. LPA advised Licensee that children should not be left for extended periods of time in furniture such as a high chairs or cribs.

LPA discussed the Community Care Licensing website www.ccld.ca.gov which will provide access to resources such as forms, regulations, Provider Information Notices (PINs), and Quarterly Updates. LPA discussed Reporting Requirements as outlined in the regulations (Section 102416.2).

Supervision of children was discussed with Licensee, and she understands that she or a qualified adult must be present in the home during day care hours and ensure that the children are supervised at all times. Licensee understands her capacity options and she understands that she cannot have more than 14 children in the home at any time and a qualified assistant must be present. Licensee understands in absence of a helper her license capacity is reduced to 8 and ratio (age of the children) must be observed.

LPA discussed "zero tolerance" related regulations which includes the assessment of an immediate $500 per day civil penalty for any violation of a "zero tolerance" related regulation. These include absence of supervision, accessible bodies of water, accessible firearms, refused entry of licensing staff, presence of an excluded person, and violations that result in illness or injury. An ongoing civil penalty of $100 per day continues until the violation(s) is corrected. Licensee was encouraged to frequently visit our website at www.ccld.ca.gov for licensing updates and regulations.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. LPA observed that the epinephrine auto injector for one child, C6 had expired. Licensee stated that new medication was provided by the parent without the original container. LPA reminded Licensee that all expired medication needs to be returned and medication should only be accepted in its original container with an unaltered label. Medication shall be administered in accordance with the label directions as prescribed by the child's physician. Parent of C6 dropped off new epinephrine auto injector in its original container along with consent form during today's inspection. Children will be isolated in the living room if necessary due to illness or communicable disease until parent pick up. For IMS information see PIN 22-02-CCP. Licensee has an IMS plan on file. Licensee states that she does not transport any day care children. LPA reminded Licensee that if she decides to transport children, children should not be left unattended in parked vehicles and that car seats shall only be used for transportation and shall not be used for sleeping.

Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. During the exit interview, Licensee confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.

Per Title 22, Division 12, of the California Code of Regulations, one Type B deficiency was cited. Appeal rights provided to Licensee. Exit interview conducted and report was reviewed with the licensee, Yolanda Lepe.
