13 | Licensing Program Analyst(LPA)Anna Morales conducted a unannounced complaint investigation today and met with Director Yareli Martinez to deliver investigation findings.
During the course of this investigation, LPA conducted interviews with staff, parents and reviewed supporting documentation.
Based on the information obtained, Facility did report an outbreak in a timely manner to the parents and posted on the front of the classroom doors an Exposure Notice in May 2025, for Hand, Foot and Mouth disease. Facility, also, posted an Exposure Notice for December 2024, for Respiratory Syncytial Virus(RSV). However, Facility failed to report two or more cases of Hand, Foot and Mouth disease in May 2025, and Respiratory Syncytial Virus(RSV) in December 2024 to Community Care Licensing.
