25 | On 11/18/2024 at 9:20am, Licensing Program Analyst (LPA) Jialing “Julianne” Zhu conducted an unannounced case management inspection for the purpose of investigating a reported incident. LPA met with Executive Director Carol Miller and Program Coordinator Colleen Coensgen. (Director Christina De Vera was not on-site during today's inspection.) Also present during the inspection were eight (8) fingerprint-cleared staff and 33 children in care. The facility is within ratio and in compliance with capacity regulations today. The facility was toured to conduct a Health and Safety Inspection.
The incident occurred on 11/07/2024 and was reported to Licensing on the same day. On 11/07/2024 at approximately 12 noon, one (1) child was outside the classroom unsupervised for approximately 3 minutes after teachers and children transitioned inside the classroom from the playground. A classroom teacher noticed later the child was not inside the classroom, and the child was found outside the classroom by the door.
During today's inspection, LPA reviewed the above incident with Executive Director and conducted an interview to obtain more information. LPA observed the play yard to be secured. The classroom door was closed during the incident. The top potion of the door is equipped with a window, but visual supervision of the child was obstructed when staff were inside the classroom. Director was notified of this incident by staff immediately. The following actions were taken by the director after she was notified of the incident: notified parent of child involved, spoke with staff involved to discuss and reflect on the incident, completed retraining of practices and procedures of transitions with staff. Director will address this incident with all staff at a later time. LPA will follow-up with Director Christina De Vera via telephone call or email as she is not on-site during today's inspection. Based on the obtained information and LPA’s observations, LPA determined there was no visual supervision of the child, which posed an immediate threat to the health and safety of the child in care. Absence of supervision is an Immediate $500 violation. As a result, a Type A deficiency is issued and a civil penalty of $500 is assessed during today’s inspection. (See LIC 809D.) Page 1 of 2.
