On 04/16/2024 at 10:30AM, Licensing Program Analyst (LPA) Jaleesa Jackson conducted a Case Management - Incident Visit. LPA met with the Director Kate Keily and explained the nature of the visit. Present on this visit were 12 Staff and 114 preschool aged children. The facility operates from Monday to Friday 7AM to 6PM.

LPA Jackson conducted an unannounced case management visit to follow up on a self reported unusual incident. LPA Jackson toured the facility and conducted interviews.

No deficiencies were cited during this visit for the unusual incident.

Notice of site visit was provided and must remain posted for 30 days.

Exit interview conducted and report was reviewed with Director Kate Keily.
