25 | On 09/28/2023 at 9:30AM Licensing Program Analyst (LPA) Farida Raja and Licensing Program Manager (LPM) Gladys Kuizon met with Licensees Batool Afjei and Seyed Ghoreyshi for an Informal Meeting. The pre-scheduled meeting took place at the San Jose Regional Licensing Office.
On 09/15/2023 at 11:10 am, LPA conducted an unannounced facility visit and observed 6 infant children in care. The licensee and 1 assistant (S1) were present. LPA observed daycare activities being conducted in the garage. LPA observed two reclined bouncers in the garage room with an infant (C1) sleeping in one, a bouncer/rocker in the garage living area and baby walker in the on-limit family room. LPA observed that the facility pool's fencing has gaps where the panel connects to the access gate on both sides and between where two panels meet in two separate areas of the fence.
The facility was cited 2 Type A deficiencies and 9 Type B deficiencies during the visit. The facility also has a history of 1 Type A and 10 Type B citations. The purpose of the informal meeting is to review the deficiencies with the Licensees and discuss the plan of corrections submitted by the licensee to ensure licensees understanding of Title 22 violations that led to the deficiencies.
LPM Gladys Kuizon explained the informal meeting and the administrative process. Licensees were advised that continued non-compliance with Title 22 Regulations could result in their license being referred to Community Care Licensing's legal department for review and possible action against the license.
During the meeting, the Licensees was reminded that since there was an issuance of Type A citations, a copy of the Facility Evaluation Report LIC809 has to be posted in the facility and a copy must be given to all parents of current and newly enrolled children for next 12 months. In addition, a copy of LIC9224 Statement Acknowledging Receipt of Licensing Reports needs to be signed and kept in child files.
