On January 14, 2026, Licensing Program Analyst (LPA), Syeda Bahar conducted an unannounced complaint investigation at the facility. The purpose of today's visit is to deliver the findings. LPA met with the Director, Marie Nunez, and explained the purpose of today's visit.

During the course of the investigation, LPA conducted observations, interviewed staff, parents and children, reviewed records, and obtained copies of facility roster, personnel report and other supporting documents. Based on the evidence gathered, the center conducts a health check every day during children’s drop-off time. The Director stated that if teachers notice any injury, they record it on the health check forms. The Director also stated that if an injury is on the shoulder or above, they notify the parents immediately. If the injury is minor, teachers notify the parents during pickup time. Based on interviews, staff walk around the classroom and stay with the children’s group during indoor and outdoor activities. Staff immediately attend to children with injuries, provide an ice pack, bandage, and emotional support to comfort the child.

Based on observations and interviews, parents drop off the children in the classroom. There is a sign-in and sign-out sheet in each classroom. Parents sign upon arrival and at pickup. The Director stated that parents know the main entrance code and go straight to the classroom upon arrival. The center has a policy regarding late arrival and late pickup. The Director stated that parents can provide a doctor’s note for late arrival. Based on interviews and record reviews, there is a late pickup fee. Based on interviews, parents usually communicate with the Director after dropping off the children in the classrooms, if necessary.

Based on the evidence gathered at this time, it is concluded that the allegations noted on the previous page may have happened or is valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur. Therefore, the allegations are UNSUBSTANTIATED.

No deficiencies were cited during today's inspection. Appeal rights were provided. Exit interview conducted and report was reviewed with the Director, Marie Nunez.

A NOTICE OF SITE VISIT WAS GIVEN AND MUST REMAIN POSTED FOR 30 DAYS.
