Licensing Program Analyst (LPA) Marilou Monico conducted an unannounced complaint inspection. LPA met with Assistant Director, Shelly Frost, and Executive Director, Julie Kennedy. During today's visit, LPA interviewed staff, reviewed files, and deliver investigation findings.

Based on available evidence, record reviews, and interviews, it is determined that although the allegation noted on this complaint may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur. The allegation is deemed UNSUBSTANTIATED.

There were no deficiencies cited.

A NOTICE OF SITE VISIT WAS GIVEN AND MUST REMAIN POSTED FOR 30 DAYS.
