On 06/11/2026 at 10:25am, Licensing Program Analyst (LPA) Jialing “Julianne” Zhu conducted an UNANNOUNCED COMPLAINT SITE INSPECTION. LPA met with Director Baishakhi "Boi" Hazarika. LPA advised Director the nature of the inspection is to deliver the findings to the allegations of the complaint. Present during the inspection were six (6) fingerprint-cleared staff and 49 children. Throughout the course of the inspection, LPA requested and obtained a copy of the Facility Roster, current Personnel Report, and other relevant documents. LPA toured the facility inside and outside for health and safety inspection. LPA conducted interviews with Executive Director, Director, a sampling of teachers, and a sampling of parents. On 04/24/2026 at 11:50am, LPA observed the outdoor play area had 3 teachers with 38 children after another teacher brought 3 children inside to use the restroom. During today's inspection at 11:25am, the facility was in ratio (5:43) in the outdoor play area. Based on LPA’s observations, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. California Code of Regulations 101216.3(a) is being cited on the attached LIC 9099D. A Notice of Site Visit was given and must remain posted for 30 days. Exit interview was conducted, report was reviewed, and Appeal Rights were provided to Director Baishakhi "Boi" Hazarika.

Although the above allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are unsubstantiated.
