25 | Licensing Program Analyst (LPA), Shine Yu, conducted an unannounced Annual Inspection 12:45 pm and was greeted and met with the Licensee, Guan Jian.
LPA observed the required posted materials, including the Facility License, Emergency Disaster Plan (LIC610), and Parents' Rights (PUB 393). Facility's operating days and hours are Monday to Friday 8:00 AM to 6:00 PM. LPA observed a current Children's roster. The last fire drill was conducted on 04/15/2025. Present are licensee, licensee's spouse and 11 children, including 4 under 2 years old.
LPA reviewed a random selection of 6 children's files and observed that notification of parent's rights forms, immunization records forms, consents for emergency medical treatment forms, and Identification forms are in each file. Licensee records the 15 minutes check log in his computer. LPA reviewed and verified the records.
LPA also reviewed the staff files for the following records: Employee Rights (LIC9052), Statement Acknowledging Requirement to report Child Abuse (LIC9108), and Immunization Record showing immunity to measles (MMR), pertussis (Tdap), and a statement of declined influenza shot. Both Licensees' pediatric 1st Aid and CPR certificates, expire on January 2027. Licensee's mandated reporter training certificate expired on May, 2027.
Licensee accompanied with LPA to tour the facility. Off limits areas inside the home are bedroom 3, 4, 5, 6 and adult bathroom. LPA observed the fire extinguisher (2A10BC) in the entrance hall way, the smoke and carbon monoxide detector are in family room and kitchen. LPA provided a copy of Healthy Beverages in Child Care flyer(AB2084) to the licensee. The bathroom is clean and diapers are organized on the rack. There are age-appropriate toys in the family room and play yard for children. Licensee stated there is no firearm in the facility. There is no stairs in the facility. There is no body of water observed outside of play yard. The outdoor play area is enclosed with appropriate fence and safe for children.
